In primary care, Patient-Centered Medical Home (PCMH) is defined as "Design of patient care wherein institutional resources and personnel are organized around patients rather than around specialized departments. ."[1]
Effectiveness
A systematic review found that effectiveness is increased if the PCMH is within an integrated delivery and finance system[2].
