| Standardized protocol is used to identify individuals, who may be at risk for [[suicide]]. Various scales have been used to predict the suicide risk. Some of them being Columbia suicide severity rating scale (C-SSRS), Becks scale for [[suicidal ideation]]

| |
| Although screening assessment for [[suicide]] risk is only a part of the broader aspect of [[suicide]] risk assessment, a number of scales are available:

| * Columbia suicide severity rating scale(C-SSRS)- This is a questionnaire used for [[suicide]] assessment
| |
| * Beck's scale for suicidal ideation
| |
| * Use of Columbia 's standardized classification algorithm led to a 50% reduction in suicide attempt
| |

| * 1-5 rating for [[suicidal ideation]]:( From a wish to die to an active thought of killing oneself with plan and intent:

| Q.1 Have you wished you were dead or wished you could go to sleep and not wake up
| |

| |
| Q.2 Have you actually had any thoughts of killing yourself
| |
|
| |
| If answer is "no" to both, no more questions on [[Ideational dyspraxia|ideation]]

| If " Yes" to active thoughts, then ask the following three questions:

| Q.3 Associated thoughts of methods. e.g.- Have you been thinking about how you might do this?

| Q.4 Some intent-Have you had these thoughts and had some intention of acting on them?

| |
| Q.5 Plan and intent- Have you started to work out (or) worked out the details of how to kill yourself? Do you intend to carry out this plan?
