ADT feeds may be coordinated by a regional Health information exchange (HIE).
Notifying the primary care physician at the time of admission, using the hospital's HL7 admission, discharge, transfer (ADT) message or feed may reduce hospital readmissions[2].
ADT feeds are well-received by health care providers[3][4].
