Overview

The differential diagnosis of diabetic nephropathy includes a spectrum of nephrotic and nephritic renal diseases and other systemic diseases with renal manifestations. The diagnosis of diabetic nephropathy should not be directly presumed on the basis of presence of renal disease in a diabetic patient. The gold standard for the diagnosis of diabetic nephropathy is renal biopsy.

Differential Diagnosis

The differential diagnosis of diabetic nephropathy includes the following:

• Nephroangiosclerosis
• Membranous glomerulonephritis
• Membranoproliferative glomerulopathy
• Lupus nephritis
• IgA nephropathy
• Henoch-Schonlein purpura
• Renal amyloidosis
• Multiple myeloma and other gammopathies
• Cryoglobulinemia
• Polycystic kidney disease
• Atherosclerotic renal artery stenosis
• Fibromuscular dysplasia
• Interstitial nephritis
• Renal vein thrombosis
• Renovascular hypertension
• Urinary tract disorders, such as infection, obstruction, or stones
