Presents in 30% of patients with Waldenström's macroglobulinemia.[1]
Symptomatic:
Constitutional symptoms
- Weight loss[1]

- Night sweat
- Lymphedema
Hyperviscosity syndrome
Accumulation of IgM proteins in blood can increase the viscosity of blood, slowing down the flow of blood to different organs. Symptoms of hyperviscosity include:[1]
- Retinal hemorrhage with blurring of vision problem
- Vertigo
- Dizziness
- Headache
- Oronasal bleeding
Bone marrow infiltration

History and Symptoms
Patients with WM may present with various symptoms.[1]
- Approximately 30% of the patients are asymptomatic (smoldering WM).
- Anemia with mean hemoglobin less than 10 g/dL - fatigue
- Bone marrow infiltration: cause anemia & pancytopenia
- Extramedullary infiltration - hepatomegaly, splenomegaly, lymphadenopathy
- Hyperviscosity - oronasal bleeding, retinal hemorrhage with blurring or loss of vision, headache, vertigo, ataxia
- IgM monoclonal proteins - peripheral neuropathy
