Bone marrow (BM) aspirate, peripheral blood and trephine biopsies are the gold standard test for the diagnosis of hairy cell leukemia.

Peripheral blood smears is one of the gold standard test for the diagnosis of hairy cell leukemia.
The following result of peripheral blood smears is confirmatory of hairy cell leukemia:[1]
- Oval nuclei and abundant cytoplasm with "hairy" projections
- Fried egg appearance
- Loose chromatin
- Mast cells numbers elevation
- Extravasated red cells are positive in peripheral blood smear of hairy cell leukemia patients
