Overview
Orbital cellulitis must be differentiated from various conditions affecting the eye and presenting with similar symptoms. Differentials include infectious disease, vascular abnormalities, structural abnormalities, autoimmune disorders, drug toxicities, and neoplasia.
Differential Diagnosis
Orbital cellulitis may share common features or present similarly to other eye diseases with significantly varying management protocols. As such, the clinician must carefully consider the patient's history, the presenting signs and symptoms, and cross reference the following differentials:[1][2]
Infectious diseases
- Periorbital cellulitis
- Endophthalmitis
- Severe conjunctivitis
- Herpes Zoster Ophthalmicus
- Posterior Scleritis
- Mucormycosis, Aspergilliosis of the globe
- M.Tuberculosis of the globe
Vascular
- Cavernous sinus thrombosis
- Atriovenous fistula
- Orbital varices
Autoimmune/Inflammatory
- Exophthalmos
- Wegeners granulomatosis
- Sarcoidosis granuloma
- Orbital pseudotumor
Structural/Trauma
- Retrobulbar hematoma
- Mucocele
- Dermoid cyst
Toxicity
- Bisphosphanate use
Neoplastic
- Metastatic carcinoma
- Neurofibroma
- Schwannoma
- Glioma
- Retinoblastoma
- Rhabdomyosarcoma
- Neuroblastoma
- Lymphoma
- Leukemia
- Histiocytosis
