Examine the patient: ❑ Vital signs: ❑ Blood pressure: ↓ ❑ Pulse rate: ↑ or ↓ ❑ Respiratory rate: ↑ or ↓ ❑ Oxygen saturation: ↓ (<90%) ❑ Temperature: ↑ or ↓ in sepsis ❑ Skin: ❑ Pallor ❑ Jaundice ❑ Petechiae, ecchymosis ❑ Active bleeding ❑ Central nervous system: ❑ Altered sensorium ❑ Personality changes ❑ Lungs: ❑ ❑ Abdomen: ❑ Abdominal distension ❑ Abdominal tenderness
