Admission, discharge, and transfer system
In health informatics, Admission, discharge, and transfer system (ADT), "The Patient Administration standard defines the requirements and specifications to support the interoperability among clinical and non-clinical systems regarding patient encounters and administrative registries."[1]
Notifying the primary care physician at the time of admission, using the hospital's HL7 admission, discharge, transfer (ADT) message or feed may reduce readmissions[2]
