Inclusion Criteria

Does the patient meet ANY of the following criteria?	Hospitalized > 96 hours	Yes	No
	Body weight < 47 kg	Yes	No
	Unable to receive nourishment (oral, NG)	Yes	No
	Life expectancy < 8 weeks	Yes	No

Does the patient meet ANY of the following criteria of high bleeding risk?	Significant GI, pulmonary, or GU bleeding, chronic peptic ulcer disease or gastritis	Yes	No
	Clinically significant bleeding within 6 months	Yes	No
	Any IC bleeding, cancer, trauma or lesions within 3 months	Yes	No
	Received fibrinolysis or thrombolysis in prior 30 days	Yes	No
