Overview
In PERFUSE Grade 3 thrombus, moderate sized thrombus is present which in its greatest linear dimension is greater than 1/2 but less than 2 vessel diameters. [1]

Shown below are an animated image and a static image depicting PERFUSE grade 3 thrombus. Outlined in yellow in the image on the right is the thrombus. Note that the linear dimension of the thrombus is less than two vessel diameters.
