The majority of patients with familial adenomatous polyposis are asymptomatic till colorectal cancer happens. Common symptoms of familial adenomatous polyposis are gastrointestinal bleeding, pain, and altered bowel habits. They might have fatigue following occult bleeding.

Patients with familial adenomatous polyposis may have a positive history of:
- Family history of similar condition
- Family history of colorectal cancer
Common Symptoms
Common symptoms of familial adenomatous polyposis are:[1]
- Gastrointestinal bleeding
- Abdominal pain
- Diarrhea
- Altered bowel habit
- Tenesmus
- Obstruction
- Fatigue due to iron deficiency anemia
- Weight loss due to colorectal cancer
Extracolonic manifestations
Patients with familial adenomatous polyposis might have extracolonic manifestations including:[2]
- A swelling or lump due to desmoid tumors of the abdominal cavity which might be with or without pain
- duodenal adenomas 50-90%
- carcinomas
